For those volunteers interested and scheduled to drive golf carts on the OLPH campus for a specified event, please complete the form below in advance of your scheduled volunteering date.

If you have not already done so, please see the OLPH office so that we may obtain a copy of your driver's license.  You may do this on the day of the scheduled event.
